Based on the guidelines and regulations provided by the state, DMC is now able to offer COVID-19 vaccinations to the following people:
Those 65 years of age and older
All healthcare workers
Front-line essential workers in critical infrastructure:
Pre-kindergarten through high school teachers, support staff and child care workers
First responders (firefighters, police, conservation officers, inspectors)
Corrections workers (staff in prisons, juvenile justice facilities)
Workers in homeless shelters, congregate child care institutions, and adult and child protective services
If eligible, please remember to bring the following to your appointment:
A form of identification verifying your date of birth
Proof of insurance (if applicable)
Proof of your employment as an essential worker
You may request a COVID-19 vaccination by filling out the form below; only one person per form. Once complete, our schedulers will follow up with a phone call if you meet the eligibility requirements at this time.
