Beginning February 28th 2017 at 6:00AM the on-line form for a Problem Ticket or Service Request has been removed. To log a Problem or Service Request you can either:

  1. Call the Service Desk at (313) 966-2400
  2. --OR--

  3. Send an email to tenet.helpdesk@dell.com. Please include the following:
  4. a) Your first and last name
    b) Location
    c) Department or clinic
    d) Phone number
    e) Email address
    f) User ID
    g) Asset Tag (if hardware related)
    h) Building
    i) Floor
    j) Room#
    k) Thorough description of the problem or request.
