COMMERCE P.T. FITNESS PROGRAM
The Fitness Program is open to former physical therapy and occupational therapy patients for up to one year following discharge of therapy. The purpose of this program is to allow former patients to continue use of exercise equipment on an independent basis, in an attempt to further improve or maintain function. You are eligible to join the Fitness Program within one month of discharge from therapy. No former patient will be admitted to the program unless he/she joins within that time frame.

Only people who present a medical clearance from their physician will be permitted to join the fitness program. You need to obtain this clearance just prior to your discharge. Any changes in your medical status, while in treatment or during enrollment in the Fitness Program, must be reported to a therapist immediately.

As a member of the Fitness Program, you are no longer considered and active patient. You must also be independent in your exercise program. If you notice a return of your symptoms, you are to contact your physician and make an appointment to see him/her.

ALL ACTIVE PATIENTS HAVE PRIORITY ACCESS TO EQUIPMENT IN THE GYM. WHENEVER A THERAPIST INDICATES THE NEED TO USE THE EQUIPMENT YOU ARE USING, YOU ARE TO GET OFF AT THAT TIME. YOU MAY RESUME ONCE THE EQUIPMENT IS AVAILABLE AND NO OTHER PATIENTS NEED TO USE IT.

Do not use any equipment for which you have not previously received formal instruction. When not used safely, all our equipment has certain risks to your health.

We ask that you wear appropriate dress while exercising (i.e. shorts, t-shirt, sneakers, etc.). Changing rooms and a shower are available to you. During the winter months or inclement weather, please bring a change of shoes to avoid excessive wear to the equipment and facility.

We kindly request that you do not bring any children. The equipment in our gym may potentially be hazardous to children.

Cost: $40/month, or $20/month for seniors 65+, payable at the front desk the beginning of each month. Payment can be made by check, money order, or charge. Cash is not accepted.
