EMPLOYMENT PROCESS STEPS

  1. Contact your site Occupational Health Services Department to schedule your physical.

  1. Complete all paperwork in the attached packet and bring to Human Resources meeting at your site.

    MI-W4 Michigan Tax Form

    Federal W-4 Tax Form

    I-9 Employee Eligibility Verification Form
 

Bring all required documents as well.  This includes evidence of all education and credentials required for your job.  Also, ensure that all your education, training, work experience and credentials are completely and accurately reflected on the application, as this will be used to verify you meet the minimum qualifications for your position. 

Review the Compliance Manual that your supervisor will give to you, and sign the Certification Form in the forms packet. Click here to review online or print the 156-page Compliance Reference document.

  1. Attend a Human Resources Onboarding session (See schedule).  

4/17–5/3

  1. Complete NetLearning (online required training modules).  New employees must complete their NetLearning modules prior to beginning employment. Instructions for completing your NetLearning modules from home/non-DMC computer are below, along with a link to the online NetLearning Site. All DMC Staff are required to complete the Employee Education Netlearning Modules annually. See your manager for your employee number.

  1. Complete all parts of the Pre-Employment Physical, as the physical may require you to visit OHS more than once.

  1. Schedule a call with a Benefits Counselor.  See Onboarding web page for details. DMC Benefits & Enrollment Info Sheets:

    5/17– 5/24

    1. Complete your scheduled call with a Benefits Counselor.

    5/22–5/31

    1. Your ID badge form will be given to you by your manager.  You are to get your badge the week you get the form. 

    5/30

    1. Attend System New Employment Orientation – your director will schedule you for a session.  The HIM team will attend in small groups in June–September. 

    June – Sept.


    Complete as many forms as possible prior to your site HR meeting. If you have questions before your site meeting call (313) 578-3185.


    REMOTE STAFF
    If you will be traveling to A DMC campus for your on-boarding -
    If you live within a reasonable distance of a DMC campus, we ask that you come on-campus for a day to complete your drug screen (indicate “drug screen only” when scheduling OHS appointment)  and HR Onboarding session. 

    If you will be completing your on-boarding remote –

    If you do NOT live within a reasonable distance of a DMC campus, you will complete your on-boarding remote, and you will need to complete the following steps:

    • Contact Athena Hinojosa, ahinojos@dmc.org, 313/578-3989, for a remote drug screen referral.
    • Secure the services of a Notary Public to complete the I-9 work eligibility form.
