The Detroit Medical Center (DMC) is an integrated academic multi-center health care system with over 2,000 hospital beds serving the people of southeast Michigan. The health center is composed of seven hospitals, two nursing centers, and approximately 150 outpatient facilities. Our 100-acre Central campus in the heart of Detroit, is the hub of the DMC consisting of Children's Hospital of Michigan, Detroit Receiving Hospital and University Health Center, Harper University Hospital/Rehabilitation Institute of Michigan, Hutzel Hospital, and Heart Hospital. The Northwest region contains Sinai-Grace Hospital and the Oakland region contains Huron Valley-Sinai Hospital.
The Detroit Medical Center Department of Pharmacy Services provides a broad spectrum of services across all hospitals of the DMC. Complemented by an established affiliation with Wayne State University's Eugene Applebaum College of Pharmacy and Health Sciences and its School of Medicine, pharmacy practice is progressive and dynamic integrated with research and education. A strong collaborative model exists between each hospital pharmacy department in the medical center. This long and recognized history of partnership is an integral component of a broader professional relationship with the medical staff and other health care professionals.
Our Pharmacy Practice Model
As reflected in our departmental mission statement, the provision of quality pharmaceutical care is among the top priorities of the DMC Department of Pharmacy Services. To fulfill this mission, the DMC employs well-trained clinical pharmacists and clinical pharmacist specialists. Our clinical pharmacists and specialists provide patient care in many areas of practice, where they work closely with physicians, nurses, therapists, and most importantly, our patients, to prevent, as well as identify and resolve medication-related problems. We use technology and pharmacy technician support in innovative ways to maximize pharmacist involvement in numerous clinical practice areas. Pharmacists at the DMC participate in patient care activities for a variety of patient populations ranging from the neonate to the geriatric. The DMC pharmacist is an integral part of the health care team at each of our hospitals.

The Detroit Medical Center Department of Pharmacy Services and DMC Geriatric Center of Excellence were recognized by The American Society of Health System Pharmacists (ASHP) with a Best Practice Award for 2013. The department was honored for its program to provide advance pharmacy practice services in post-hospitalization transitions of care. This collaborative pilot program was initiated at the Rosa Parks Geriatrics Clinic and has an integrated inpatient component which is being implemented across the DMC. The DMC pharmacist-driven medication reconciliation model is focused on high-risk patients across the continuum of care.
